Assisted fertilization

In addition to a medical indication, a stable relationship is required to qualify for treatment.  Another requirement is that the couple should be able to take care of any child who may result from treatment, throughout childhood.

The woman must have a reasonable chance of pregnancy, and pregnancy must not be contraindicated. Maximum age is 40 for women and 55 for men. For medical reasons, the woman should have a BMI <30 and must not have a BMI >35.  Smoking reduces the chance of pregnancy.
